While the digital Advance Care Plan is electronic, MyDirectives® users can print a copy for use when needed.
1. Sign into your MyDirectives account at www.app.MyDirectives.com
(if you cannot remember your password, you can reset it by clicking on the "Forgot Credentials" link)
2. On the My Dashboard screen, click on the Print Plan tile in the top right-hand corner of your screen.
3. Clicking this will take you to review the document you wish to print, once you have the document you wish to print, click on the blue Print link.
4. Follow your computer prompts to print your document to your home printer.
