We encourage our users to review your Advance Care Plan regularly to ensure that your plan reflects your current preferences. If at anytime you need to update your plan, please follow the below steps to do so.
What you will need
- The Username for your MyDirectives® account
- The Password for your MyDirectives® account
Steps to take
- Sign into your MyDirectives account at www.app.MyDirectives.com
(if you cannot remember your password, you can reset it by clicking on the "Forgot Credentials" link) - On the My Dashboard screen, click on the Update Plan link to be taken to your Advance Care Plan page.
- Here, you will be able to view the different topics you can update within your Advance Care Plan. If you wish to start from the beginning, click on the Update button in the first topic: What Matters Most to Me.
Once you have made all the changes you want to record, please be sure to sign the new version of your Advance Care Plan so that it is accessible by those you shared it with, and any medical teams that might need to reference it to provide treatment tailored to what is more important to you.
