Once you have digitally signed your Advance Care Plan, you will be able to collect your witness signatures from witnesses present with you by following the below steps:
Starting in your dashboard, click on the My Advance Care Plan button on the left-hand side of your screen.
Note – Witness signatures can only be collected the same day that your Advance Care Plan is signed. If you are beginning this process on another day, click on the Unsign/Re-sign my ACP link in your dashboard to reflect today’s date.
On the My Advance Care Plan page, click on the Add Witness and/or Notary to your Plan button to be taken to the Witness/Notary page.
Since we are collecting witness signatures on your screen, we will navigate to the bottom of your screen under Option 3. Enter the information for your witness(es) and have them sign on your screen by typing or drawing.
Note – As your witnesses are signing in your presence, the only information that is required to provide is their name and signature.
Click the blue Submit button to have their signatures reflected on your Advance Care Plan.
