Creating an account on MyDirectives is quick and easy.
What you will need
- Access to a web browser (desktop, tablet, or mobile)
- Your Name, Date of Birth, Gender, and an Email Address and/or Phone Number
Steps to take
- In your browser, navigate to www.app.MyDirectives.com and select the New User button in the upper right of the screen, or click here to link directly to the New User page
- Enter your contact information and click the blue Sign Up button
- Your Username may be your email address
- If not your email address, your Username will need to conform to the following criteria:
- Your Password will need to conform to the following criteria:
- The next page will explain that you will need to verify your account through a code sent to your email address or phone number depending on the information you provided. A 4 digit code will be sent for you to verify your account.
- Once your account has been verified, you will be taken to your dashboard and will begin the tour of our site and how to navigate it.
- From here you can continue:
- Creating your digital advance care plan by clicking the Start My ACP link
- Complete your account by clicking on the Update My Profile link
- Sign out by click the User Icon button in the upper right, then the Logout button
In step 2, you can choose to connect to MyDirectives® with Apple or Facebook instead of filling out the information directly into MyDirectives®.
