Creating an account on MyDirectives is quick and easy.


What you will need

  • Access to a web browser (desktop, tablet, or mobile)
  • Your Name, Date of Birth, Gender, and an Email Address and/or Phone Number


Steps to take

  1. In your browser, navigate to www.app.MyDirectives.com and select the New User button in the upper right of the screen, or click here to link directly to the New User page

  2. Enter your contact information and click the blue Sign Up button
    • Your Username may be your email address
    • If not your email address, your Username will need to conform to the following criteria:Password requirements
    • Your Password will need to conform to the following criteria:
  3. The next page will explain that you will need to verify your account through a code sent to your email address or phone number depending on the information you provided. A 4 digit code will be sent for you to verify your account.
  4. Once your account has been verified, you will be taken to your dashboard and will begin the tour of our site and how to navigate it.
  5. From here you can continue:
    1. Creating your digital advance care plan by clicking the Start My ACP link
    2. Complete your account by clicking on the Update My Profile link
    3. Sign out by click the User Icon button in the upper right, then the Logout button


In step 2, you can choose to connect to MyDirectives® with Apple or Facebook instead of filling out the information directly into MyDirectives®.
