Creating an account on MyDirectives is quick and easy.
What you will need
- Access to a web browser (desktop, tablet, or mobile)
- Your Name, Date of Birth, and an Email Address
Steps to take
- In your browser, navigate to www.app.MyDirectives.com and select the New User button in the upper right of the screen, or click here to link directly to the New User page
- Enter your contact information and click the green SUBMIT button
- Your Username may be your email address
- If not your email address, your Username will need to conform to the following criteria:
- it can contain letters (a-z), numbers (0-9) or special characters !#$%&*+-/=?^_`{|}~.
- be at least 8 and maximum 50 characters long
- have no spaces
- have at least one upper or lowercase letter
- can not start/end with a period, or contain multiple periods in a row
- The next page will explain that you that you have been sent an Email Confirmation. Check the inbox for the email you entered on the previous step and click the Continue button (or follow the in email instructions) to confirm your email address
- On the Account Confirmation page:
- Review the information under the My Contact Information, no action is needed if the information is correct, but if you would like to change any information click "Edit" next to My Contact Information
- Move down to the next section Create a Password and enter a Password that meets the criteria noted on the screen, then enter the same password under Confirm Password
(You can click "Show Passwords" to unhide the characters and confirm the passwords entered) - If you were given an Enrollment Code you can enter it under the Enrollment Code section.
- Before completing the Account Confirmation you will need to agree to the Terms and Conditions of Use by check the box
- Review the information under the My Contact Information, no action is needed if the information is correct, but if you would like to change any information click "Edit" next to My Contact Information
- Once the information has been completed, click the large green SAVE AND CONTINUE
- You will receive a welcome email where no action is required, and the web browser page will refresh to a Welcome to MyDirectives notice. From here you can continue:
- to creating your digital advance care plan by clicking the large GET STARTED button;
- to your dashboard by clicking the My Dashboard menu option in the top blue menu; or
- sign out by click the SIGN OUT button in the upper right.
In step 2, you can choose to connect to MyDirectives® with Apple or Facebook instead of filling out the information directly into MyDirectives®.
